Force comments on corrective actions (so that team members provide adequate detail)

For some exceptions, it's vital that the corrective action is detailed properly when a team member raises it. Currently, team members do the bare minimum when raising a mandatory action. Having the ability to set certain actions to require a comment should ensure we get the detail we need to know what the plan of action is and can intervene if need be. Action templates should also help with this.
